Compassionate Dementia Care
Alameda Care Center is a secured skilled nursing facility with 89 beds, designed to support residents living with Alzheimer’s disease and other forms of dementia. We provide a safe, structured environment with personalized care plans and professional support for activities of daily living.
Our Dementia Program focuses on each resident’s strengths, supporting what they can do rather than focusing on what they can no longer do. Residents receive 24-hour nursing care, assistance with activities of daily living such as dressing, hygiene, and meals, and are encouraged to participate in a structured activity program offered seven days a week.
Care plans are reviewed and updated as needs change to ensure residents continue receiving the appropriate level of support with compassion, dignity, and professionalism.
Our admission policy is 7 days a week, and we are now accepting referrals. We welcome you to tour our facility and see firsthand how Alameda Care Center provides a supportive, family-centered environment for your loved one.

Alameda Care Center provides 24-hour nursing care with ongoing monitoring for safety and changes in condition. When changes occur, our team communicates promptly with the resident’s physician to support appropriate evaluation and follow-up care.
Residents receive daily meals and snacks, assistance with activities of daily living, and rehabilitation services when needed, all delivered with compassion, dignity, and professionalism. Families are welcome to visit and remain involved in their loved one’s care.
