Alameda Care Center is a secured skilled nursing facility with 89 beds located in the heart of Burbank. We specialize in the care of Alzheimer’s and Dementia providing a safe, and structured environment with personal and individualized care. While in our community, your loved one will be cared for in the most compassionate and professional manner. As the disease progresses and their abilities change, his or her plan of care will change as well. Our Dementia Program focuses on their abilities – what they can do rather than what they are unable to do.
While at Alameda Care Center, your loved one will have 24 nursing care and be monitored for safety and any changes in condition. When changes occur, that information is immediately relayed to your loved one’s doctor for further evaluation and instructions. In the event your loved one needs to be hospitalized; we are located just 1 mile from St. Joseph Medical Center.
Daily life for your loved one includes breakfast, lunch, and dinner, with snacks throughout the day. If needed, they will have rehabilitation services located in our community either in our rehabilitation room or in their room if necessary. Your loved one will also be encouraged to participate in our specialized activity program which is provided 7 days a week at various times. If needed, our nursing staff will also help with activities of daily living which include but are not limited to: dressing, hygiene care, showering and assistance with feeding.
Alameda Care Center is proud to be rated 4 stars by the Centers for Medicare & Medicaid Services (CMS). We are one of few skilled nursing facilities with a 4-star rating and are always looking for ways to improve our quality of care. This is achieved through training throughout the year and by working with our partners at local hospitals and community groups who specialize in Alzheimer’s and dementia care.
